PO-05-055 A META-ANALYTIC COMPARISON OF OUTCOMES OF LEFT BUNDLE BRANCH AREA PACING (LBBP) VERSUS BI-VENTRICULAR PACING FOR CARDIAC RESYNCHRONIZATION THERAPY

چکیده
Left bundle branch area pacing (LBBP), has emerged as a novel method to deliver cardiac resynchronization therapy (CRT), an alternative biventricular (BVP). We aimed evaluate the outcomes of LBBP BVP CRT. A systematic literature-based search was performed on various electronic databases, i.e., PubMed, Embase, Scopus, and Cochrane libraries, for studies that compared with BVP. The primary were studied changes in left ventricular end-diastolic diameter (LVEDD), New York Heart Association (NYHA) classification, ejection fraction (LVEF) paced QRS duration post CRT device implantation. Six articles (1 randomized trial five observational studies) total 399 patients (42% CRT) included final analysis. pooled random-effects analysis revealed who underwent had significantly greater reduction [Mean Difference (MD) 0.40 (95% Confidence Interval (CI), 0.17-0.63); p < 0.001] [MD 1.03 CI, 0.63-1.43); group. There also improvement NYHA class 0.64 0.33-0.96); LVEF 0.83 0.33-1.33); (Figure 1 forest plots) potential improve clinical over conventional undergoing This finding should be further confirmed by trials larger sample size well-defined outcome measures feasibility indicators. Although limited nature studies, our meta-analysis sets stage towards consideration meet indications

Left ventricular endocardial pacing improves resynchronization therapy in canine left bundle-branch hearts.
BACKGROUND We investigated the benefits of the more physiological activation achieved by left ventricular (LV) endocardial pacing (ENDO) as compared with conventional epicardial (EPI) LV pacing in cardiac resynchronization therapy. METHODS AND RESULTS In 8 anesthetized dogs with experimental left bundle-branch block, pacing leads were positioned in the right atrium, right ventricle, and at 8 ...
متن کاملAlternative techniques for left ventricular pacing in cardiac resynchronization therapy.
Cardiac resynchronization therapy (CRT) is an important treatment modality for a well-defined subgroup of heart failure patients. Coronary sinus (CS) lead placement is the first-line clinical approach but the insertion is unsuccessful in about 5-10% of the patients. In recent years, the number of CRT recipients and the considerable need for left ventricular (LV) lead revisions increased enormou...
متن کاملSimultaneous His Bundle and Left Ventricular Pacing for Optimal Cardiac Resynchronization Therapy Delivery
The beneficial effects of cardiac resynchronization therapy (CRT) have been evaluated in several trials, and different degrees of response to CRT have been reported; indeed, the optimization of pacing sites and stimulation timing seems to be a major determinant of the hemodynamic response. For standard CRT delivery, the atrioventricular pacing interval is shortened to ensure consistent left ven...

Single site left ventricular pacing for cardiac resynchronization.
Chronic, dilated heart failure is often accompanied by delayed ventricular electrical activation manifested as prolonged QRS duration, most commonly in the form of left bundle branch block (LBBB). Exploration of the link between the sequence of cardiac electrical activation and mechanical function is one of the most exciting contemporary areas of research in heart failure, but recognition of th...
